Any Columbia Affiliate program which invites its minor participants to our campuses, or otherwise exercises custody and control over its minor participants, must utilize, at a minimum, the below forms. If you are running a virtual program or activity, you do not need to use these forms. If your program currently utilizes your own versions of these forms, feel free to continue their use as long as they provide and gather the same information as the forms below.
- Assumption of Risk, Waiver and Release
- Emergency contact form
- Media Release form
For Commuter Programs:
- Commuter transportation permission form
- Attendance form
All forms must be secured by the program administrators and retained for a period not less than three (3) years (e.g., uploaded to a CUIT DropBox called Box@CU). Please contact [email protected] for instructions.
All information collected must be treated in a careful and sensitive manner with appropriate restrictions and shared only with those who have a legitimate need to know in order to support the child.
Forms containing medical information of any kind must be secured separately from other student information in Box@CU to ensure confidentiality.
